Serves as a Social Work Program Coordinator, locally titled Caregiver Support Program (CSP) Manager, which provides a state-of-the-art continuum of services, education and resources to caregivers of Veterans with complex needs. Oversees service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans regardless of period of service. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Develops and facilitates CSP-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community. Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services (but who do not presently use VA for their health care needs). Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Supervises the TVHS Caregiver Support Coordinators (CSC), Program Analyst (Analyst), Program Support Assistant (PSA), the Caregiver Support Program Registered Nurses (RN), and the Caregiver Support Program Occupational Therapist (OT) which are assigned to Caregiver Support Program as outlined in VHA Directive 11152(1 ), Caregiver Support Program. Coordinates program implementation with multiple clinical services across all campus locations within the VA facility. Provides clinical interventions to the Veteran and caregiver populations served by the programs. Subject matter expert (SME) to VA Facility leadership and staff for all CSP related issues and serves as a consultant to other VA professionals and administrators on program specific activities. Performs the administrative and personnel management function relative to staff supervised including personnel actions, clinical supervision, consultation, negotiation with other departments, and quality improvement. Responsible for monitoring performance measures at the local, VISN, and Central Office levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations and other internal and external stakeholders. Oversees the daily operations of the CSP program. Develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. Plans, organizes and coordinates administrative, operational, clinical and personnel activities for the CSP program. Responsible for managing and reporting on program specific fund control points, labor mapping, workload, and clinic utilization within the assigned program area. Manages all aspects of the PCAFC and the PGCSS to ensure eligible caregivers have awareness and access to the national Caregiver Support Line (CSL), VHA's Peer Support Mentoring Program, self-care and resiliency building trainings (e.g. Building Better Caregivers), and personal care service skill development trainings (e.g., VHA Caregiver Core Curriculum) regardless of era of service.
