Edward Hines VA Medical Center is seeking a full-time Nurse Practitioner/Advanced Practice Nurse (NP/APN) Women's Health Navigator-Clinical Coordinator (WHNCC). Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) II: Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and/or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B). Scope: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others Education and Experience Requirement: Meets basic education requirement for APN (NP). Nurse II is considered the entry grade level for APN (NPs); no experience is required. Dimension Requirement: APNs (NPs) at level II are expected to meet the following dimension requirements: Clinical Practice. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) III: Scope: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Education and Experience Requirement: Meets basic education requirement for an APN (NP) AND one year of specialized APN (NP) experience equivalent to the Nurse II grade level. Dimension Requirement: APN (Nurse Practitioners) at grade III are expected to meet the following dimension requirements: Clinical Practice. Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population, or program. Outcome Focus. Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations. Leadership. Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program. Professional Development. Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility. Professionalism/Collegiality. Preferred Experience: Masters of Science Degree in Nursing (MSN/NP) OR Doctor of Nursing Practice (DNP) At least 2 to 3 Years RN, NP/APN Experience Must be have full and current board certification of clinical specialty in Women's Health (Required) Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting. ["The Nurse Practitioner/Advanced Practice Nurse (NP/APN) Women's Health Navigator-Clinical Coordinator (WHNCC) is responsible for identifying, facilitating, and coordinating patient-centered care and services. The WHNCC performs a critical role in assisting women veterans to efficiently obtain gender specific services and care, with emphasis on care coordination, coordination of gynecologic services, and monitoring and improving performance and clinical follow-up metrics. The WHNCC functions as a liaison and is responsible for planning, coordinating, and performing a wide range of clinical, administrative, program, and technical functions to support quality case management of gender-specific services for veterans. The WHNCC works directly with the patients, VA staff, non-VA care providers and organizations, Radiology Departments, and the Women Veterans Program Manager to achieve seamless care, facilitate patient satisfaction, reduce duplication of services or delay of care, and monitor quality of care and services received. The WHNCC is responsible and accountable for the provision of direct and indirect care and for the delivery of quality care to a culturally diverse Veteran population. The WHNCC possesses the knowledge and skills to effectively apply all aspects of the process with collaborative, interdisciplinary, evidence-based practice, which will enable effective care coordination and implementation of health promotion and disease prevention practices.The functions of the Women's Health Navigator-Clinical Coordinator include, but are not limited to: Coordinating and tracking gender-specific care (i.e., lipid management, breast care/mammography, gynecological care coordination, etc.) of patients throughout all aspects of the medical center, including Community Based Outpatient Clinics, as well as community vendors and/or agencies. Assisting with initial VA consult referrals for primary care, gynecology, mammography, other care, assisting with VA or community services for the patient with an appropriate provider, to include coordinating any subsequent appointments or follow-up, as needed. Educating patients on a variety of gender-specific topics, to include importance of routine medical screenings, and authorized VA and community-based resources as appropriate as needed. Assisting veterans who have presented to VA or non-VA emergency department for gender-specific care. Coordinating care with the Veterans PACT. Improving the access to care. Documenting patient care interactions accurately, succinctly, and timely, using appropriate notes, templates, and clinical reminders, in the electronic medical record. Utilizing and maintaining tracking spreadsheets and systems as required by VHA Directives and Handbooks. Managing appropriate use of the electronic alert for care coordination issues in the electronic medical record. Understanding and responding to mammography reports, BIRADS, Pap, and other gender specific screening/diagnostic test results. Communicating and documenting communication of test results for female specific testing. Contacting patients who fail to keep mammogram appointments, or who are delinquent in obtaining recommended breast cancer or other diagnostic screenings (i.e. Pap), to determine and resolve obstacles the veteran may have with keeping appointments. Maintaining professional skills in the application of professional bodies of knowledge and accepted clinical practices that will facilitate oversight of service delivery of patient care. Participating in quality improvement programs and initiatives including system redesign, to improve access, quality, efficiency, and patient satisfaction. - Provides clinical care to the veterans and also when needed acts a gap provider at the women's health clinic, when the Women's health PACT provider is on leave and covers women's health clinical care coordinator when needed.
