Summary of Job Purpose
- MDS Coordinator - The primary purpose of your job position is to complete MDS 3.0, Triggers, and RAPS for all annual, significant changes, hospital returns, and quarterly reviews, including care plan meetings. Schedules all MDS 3.0 reviews. For Medicare, coordinates Medicare admissions and discharges.

- Effectively manages conflict resolution and conducts formal personnel counseling when needed.
Operations/Financial

- Manages the financial resources of the department effectively and efficiently.
- Prepares and manages annual departmental budget, determining the needs of the department and logically communicates reasonable requests to upper management.

Human Resources

- Recruits, interviews and hires competent candidates for department position, who will be supportive of the Hospital Mission and Core Values.
- Actively works to improve the working environment to aid in the retention of staff.
- Ensures proper orientation for all new hires.
- Supports and participates in Hospital recognition programs.
- Completes Initial and Annual Performance Appraisals within the required time frames.
- Ensures staff completion of mandatory education requirements.
- Ensures staff maintains updated professional licensure and certifications and forwards documents to Human Resources.
- Actively works with Employee Health to ensure that employees adhere to DOH/DNV/Infection Control Regulations.
Staff Development

- Provides ongoing training to staff to ensure productivity and knowledge.
- Ensures employee competency by providing ongoing staff development and continuing education opportunities, including staff participation in all mandated and regulatory programs.
Quality Improvement Henry

- Implements and monitors patient safety initiatives to meet established goals.
- Reviews incident reports and patient complaints, follows up appropriately.
- Maintains target levels of customer satisfaction. Implements initiatives in the department to improve service to customers.
Essential Job Responsibilities Administrative functions:

- Assists the DON in directing day-to-day functions.
- Participates in developing, maintaining, and updating policies and procedures.
- Assists in developing methods for coordinating nursing services with other departments.
- Oversees staff compliance with job descriptions and following P/P's, and governing laws and regulations.
- Attend committee meetings/seminars as required.
- Coordinators for departments and completes nursing components of MDS as applicable. Establishes calendar of due dates and coordinates careplan (interdisciplinary conferences) meetings. Notifies applicable departments and RCCs of required MDS work to be completed.
- Takes administrator-on-call per schedule
- Leads Infection Control and reports outbreaks/reportable diseases to NYSDOH as required.
- Serves as Medicare Coordinator and works with billing to ensure Medicare eligibility.
- Serves as Medicare Part B coordinator.
Other Administrative duties:

- Participates actively with IDT and follows-through on actions determined by the Team.
- Serves as a resident advocate and ensures resident rights are maintained.
- Supports the medical staff.
- Works collaboratively with all departments at Mountainside and MMH.
- Works with staff in the planning of the unit's services, programs, and activities.
- Oversees consistency between both floors in nursing care.
- Oversees staff compliance with polices and standards of care.
- Oversees staff compliance completeness of Accident and Incident reports.
- Promotes mission and values of Mountainside.
- Serves as a leader and resource for nursing staff.
- Participates in investigations as assigned.
- Oversees that safe quality care is provided to the resident's daily, and essential information is communicated to provide appropriate continuity of care from admission to discharge.
- Is a liaison with residents, families, and other departments to adequately plan for and provide resident care.
- Guides RCC , RN Supervisors, LPN Nurse Managers, LPNs, and CNAs responsibilities.
- Oversees accurate, appropriate, and complete documentation by all nursing staff.
- Participates in general orientation of facility staff members.
Care Plan, Kardex, MDS, and Assessment

- Ensures appropriate and ongoing communication to the resident, family, staff, MD, IDT, etc.

Personnel/Staff Development

- Assists with employee evaluations, employee praise, and discipline as necessary.
- Actively participates with staff education.
- Promotes an environment of learning.
- Oversees that staff attends mandatory in-services and meets minimum continuing education requirements.
- Conducts routine staff audits to ensure care is being delivered per care plan and medical staff orders.
- Promotes work consistency among staff.
- Promotes a positive, friendly work environment.

- Oversees that appropriate are available and monitors that supplies are utilized appropriately to avoid waste.

Policy and Procedure Manuals:

- Ensures staff awareness of manuals, including other departments (e.g. MSDS, emergency protocols).
- Uses manual and assists other staff on using policies as necessary.
Quality Assurance:

- Constantly strives to enhance the resident and staff experience.
- Immediately reports unusual problem/accident/incident regarding resident care to RN/House Supervisor.
Safety/Infection Control:

- Oversees staff compliance with standards and regulations.
- Reports potential outbreaks and takes appropriate measures to control/eliminate infection control exposures.
Staffing Coverage

- Works beyond normal working hours, or in other positions temporarily when necessary (e.g. administers medications).
- Is subject to callback during emergency conditions.
- Takes RN on-call as needed.
- Assists with ensuring adequate nursing coverage for units.

Essential Job Qualifications
Education:
- Graduate of an accredited school of professional nursing as a RN
Experience:
- 2 years clinical experience
Training/License:
- NYS RN License, MDS Certified

- BLS, basic computer skills
