DEPARTMENTAL SUMMARY: The Rehabilitation Department provides therapy and nursing services to patients, families and patients caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: Provides treatment and rehabilitation under the supervision and in consultation with a licensed Physical Therapist and as prescribed by the referring physician. Works with family and team members regarding patients abilities and goals in consultation with a Licensed Physical Therapist.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Provides treatment to patients under the supervision and per established plan of care of a licensed Physical Therapist, while adhering to the Physical Therapy assistant scope of practice.
Assists the Physical Therapist with gathering pertinent data to be included in required physical therapy reassessments (for patients for whom the physical therapist assistant has been providing treatment).
Collaborates with the Physical Therapist in making modifications or changes to the plan of care or goals based on the Physical Therapy Assistants treatment of that patient, the patients condition, progress or response to physical therapy treatments.
Makes recommendations regarding discharge of patients to supervising Physical Therapist based on the Physical Therapy Assistants treatment of the patient and patient response to skilled intervention.
Provides resource information to patient, family, and staff.
Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.
Accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.
Participates in clinical care conferencing and with team members in facilitating patients and familys learning throughout the continuum of care.

EDUCATION
Required: Graduate of a Physical Therapy Assistant program from a college or university accredited by the American Physical Therapy Association.

LICENSURE/CERTIFICATION/REGISTRATION
Required: Must have necessary requisites for licensure as a Physical Therapist Assistant by the Oregon State Board of Health. Preferably a member in good standing of the American Physical Therapy Association, licensed or working under provisional permit. Valid Oregon driver's license and ability to meet SCHS driving requirements. Current CPR Certification required

EXPERIENCE
Required: Must have one year experience with therapy procedures, current orthopedic and neurological follow-up procedures, and rehabilitation practices.
Preferred: Previous Physical Therapy Assistant experience in a hospital or medical based setting.
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds.
