Under minimal supervision, the RN Supervisor is responsible for providing operational direction, supervision and oversight to the team that perform the functions of planning, implementing and managing the home visitation program model, policies and procedures. Ensures compliance with all policies and procedures, as well as, compliance with all state, federal, and regulatory requirements. Directs client and caseload management according to staff and program demands and assists in meeting key performance indicators of model by effectively assisting in managing productivity and personnel.
Essential Functions:
Supervises the daily activities of employees providing guidance and direction as needed (10%)
Serves as a resource to the healthcare team (15%)
Participates in the development, refinement, implementation and administration of the program/model, policies, procedures and standard work (10%)
Regularly monitors and evaluates compliance with performance standards (10%)
Makes staff assignments to ensure coverage to all areas (5%)
Provides daily staff oversight and direction on home visitation processes to ensure compliance with all departmental policies and processes (5%)
Coordinates ongoing staff education and training regarding current and new requirement of program and processes (5%)
Monitors staff activity and productivity (5%)
Assists in creating and interpreting monthly reports (5%)
Analyzes patterns of variance and implements strategies to improve departmental/staff performance (5%)
Provides feedback information to the staff regarding departmental and individual staff performance (5%)
Responsible for effective collaboration with internal and external resources to achieve goals and objectives (5%)
Leads productive meetings with team members. Participates on committees and workgroups to develop collaborative and integrated work processes (5%)
Ensures compliance with state, federal, and regulatory requirements (5%)
Participates in the hiring, orienting, training, performance reviews, counseling, disciplining and terminating of employees (5%)
Education:
Bachelor's Degree Completion of an accredited nursing education program that satisfies the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. Required
Work Experience:
4-6 years Five or more years of relevant clinical experience with diverse populations in a health care setting including hospital, clinic or home visitation providing patient education, case management, and/or care coordination.
Licenses:
RN-Registered Nurse - DORA - Department of Regulatory Agencies Required
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required
Knowledge, Skills and Abilities:
Strong program development, leadership and management skills.
Demonstrated ability for assessment, evaluation and interpretation of medical information, and care planning.
Possess a high level of understanding of community resources, treatment options, funding options and special programs.
Extensive knowledge of the management of clients through continuum of pregnancy, postpartum, well-person care, newborn, infant and toddler phases
Demonstrated ability to manage people and processes
Ability to work independently and be self-directed.
Demonstrates strong analytical skills
Excellent interpersonal and organizational skills.
Ability to work effectively with rapidly changing priorities and requirements with a team focus.
