The Inova Cares for Community Bridging (ICCB) clinics bridge gaps in care that patients experience as they transfer across care delivery settings between illness and recovery. The service focuses on transitional medical care and community collaboration to improve health outcomes of patients at the highest risk who lack access to needed resources.
ICCB serves adult patients and accepts insurance from all payers and uninsured patients. Patients must have discharged from Inova Hospital or ED in the last 30 days and have an internal medicine diagnosis(es) needing post-hospitalization follow-up. ICCB has a primary focus on caring for high-risk patients without established PCP or who lack prompt access to one. ICCB is equipped to manage needs within the scope of a General Internist or Hospitalist.
The Inova Cares for Community Bridging department is seeking a full-time dedicated Family Medicine, internal medicine trained physician (MD/DO) preferable with Hospitalist experience to join our team.
ICCB operates clinics in Alexandria, Fairfax, and Leesburg. This position will primarily support the Fairfax clinic with flexibility to provide coverage at all clinic sites, as needed.

Inova Cares for Community Bridging Physician Job Responsibilities:
- Provide safe and effective care to patients who have been recently discharged from hospital or ED.
- Continue preliminary workups/evaluations initiated by inpatient/ED teams.
- Be available for rapid ED discharges or early hospital discharges to help with admission prevention and hospital LOS.
- Follow up and act on any pending lab result/biopsies ordered by ED or hospital team.
- Refill meds when needed. Adjust regimen if indicated based on reassessment of patient's clinical picture as well as financial situation.
- Manage medical issues for patients that may have primarily trauma or surgical indications for hospitalization.
- Facilitate care coordination in conjunction with team in terms of specialty follow up.
- Work with team to make sure pt has a viable way to obtain meds long term and has a medical home. Work with team to secure DME, as needed.
- Address social determinates of health.
- Work with team to reduce risk of avoidable re-hospitalizations.
- Manage warfarin temporarily until patients can find a specialists or PCP to manage.
- Review and sign home health care plans and be available to HH disciplines if plan of care needs to be changed or further orders, as needed.
- Teamwork/Coordination of Care: Demonstrates ability to communicate effectively and work collaboratively with others. Exhibits respect for and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes.
- Practice/Learning: Uses evidence-based standards, guidelines, and protocols to the delivery of patient care. Incorporates new information and approaches, when appropriate, to continuously improve patient care and practice.
- Professionalism / Professional Development: Ensures ongoing growth and development of clinical skills and knowledge by assuming responsibility for lifelong learning. Provides direction and guidance to others regarding practice, also serves as a resource and mentor. Demonstrates leadership skills through participation in program development.
Additional Requirements:
- Certification - Basic Life Support upon start; Advanced Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon start
- Licensure - Licensed Physician
- Experience - Prior experience required
- Education - BE/BC in Family Medicine or Internal Medicine
