Come join our Venice Family Clinic as a Health Educator and make a positive difference in others’ lives. In this position, you will work within a multi-disciplinary health care team and report to the Director of Health Education. The Health Educator is responsible for the assessment, planning, coordination and implementation of health education programs that focus on underserved racial/ethnic groups who are at high risk for illness, disability and death from chronic diseases.
Other key responsibilities include:
Providing health education services to clinic patients in the areas of chronic disease prevention and management, including nutrition, diabetes and asthma management, and hypertension.
Assisting patients in setting self-management goals during their medical visits.
Overseeing and managing health education resources to ensure the health education materials are well stocked and readily available for use with patients.
Assisting with the development, organization and supervision of health education programs and materials for patients that seek to improve the quality of life, reduce morbidity and mortality among patients with asthma, diabetes and hypertension.
Developing and managing program tracking systems for budget management and productivity.
Monitoring and evaluating targeted intervention practices, contract requirements, and submit complete patient encounter forms in a timely fashion.
Conducting reminder and follow-up phone calls to participants of our health education programs.
Completing patient encounter forms, and tracking and reporting metrics.
Travel between clinics and community organizations to conduct health education programs, presentation, services, outreach, and other related activities.

Salary offers are determined based on various factors including, but not limited to, qualifications, experience, and equity. The full salary range for this position is $30.56 - $60.82 hourly. The budgeted salary range that the University reasonably expects to pay for this position is about $30.56 - $45.00 Hourly.
Qualifications
Required:
Master’s degree in Public Health, Nursing, Nutrition or related field OR a Bachelor’s degree in Health or a related field and at two years of experience providing health education services in a public health or primary health care organization.
Working knowledge of health education issues and programs including nutrition, lifestyle and behavioral issues.
Bilingual language skills in Spanish.
Working knowledge of chronic diseases and strategies to improve patient adherence to medical treatment plans.
Ability to establish and maintain cooperative working relationships, and work effectively as a member of an interdisciplinary team.
Excellent communication skills, and the ability to effectively communicate with a wide variety of people of various educational and socio-economic backgrounds.
Excellent organizational skills and proficiency in using Microsoft applications such as Word, Excel and Access.
Ability to travel between clinics and community organizations.
Ability to conduct outreach and recruitment for health education classes on nutrition and lifestyle changes.
