Baptist AgeWell is seeking a Full-time Nurse Practitioner to join our Homecare team. The provider must be comfortable and have experience working with adults with chronic medical conditions in a homecare setting. Proficiency with EMR required. Excellent oral and written communication skills.

• Adult-Gerontology Nurse Practitioner (AGNP) Preferred
• Psychiatric-Mental health Nurse Practitioner (PMHNP) Preferred
• Licensed Advanced Registered Nurse Practitioner Required
• Autonomous Advanced Practice Registered Nurse Preferred
