Join Our Team as a Registered Nurse - Peritoneal Dialysis Program!
Are you an experienced and compassionate Registered Nurse who thrives on teaching and empowering patients? In this role, you?ll:
?Educate & Empower Patients and Families:
You?ll take the lead in training patients and their families to perform Peritoneal Dialysis at home, ensuring they feel supported, confident, and well-prepared for their treatment.
?Monitor Treatments & Collaborate with the Medical Team:Using vendor software, you'll monitor patients? nightly treatments and collaborate directly with Nephrologists to make any necessary dialysis prescription adjustments. This collaborative approach ensures that every patient receives personalized care.
?Manage Medications: Working closely with our Nephrologists, you?ll assist patients in managing their medications, helping them stay on track and improve their outcomes.
?Provide Ongoing Support: You?ll meet with patients monthly in the clinic to check in on their progress, adjust care plans as needed, and ensure they have the resources and support to succeed.
?Ensure Quality Care: As part of the team, you?ll monitor quality metrics in line with CMS End Stage Renal Disease Conditions for Coverage to ensure we are always delivering the highest standard of care.

? A compassionate Registered Nurse (RN) with dialysis experience, preferably in Peritoneal Dialysis (PD).

? Ability to adapt to new technologies and a commitment to providing top-tier care.

MINIMUM QUALIFICATION(S):
Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact. required or Registered Nurse from another state with a Temporary PA Practice Permit required.
