Are you looking for a Clinical Pharmacist Specialist opportunity focused in Internal Medicine Transitions of Care? Our world-renowned medical team at Barnes-Jewish Hospital is seeking a Transitions of Care specialist! In this role you will provide comprehensive transitions of care (TOC) pharmacy specialist services for patients at high risk of readmission and/or patients that receive outpatient care in the Barnes-Jewish Hospital (BJH) Primary Care Medicine Clinic (PCMC) that are admitted to the medicine or cardiology services at BJH.
If you have previous internal medicine or transitions or care experience in a hospital, clinic, or academic setting this may be the opportunity for you! PGY2 residency preferred.
M-F – days
2 weekends a year rotation

Here, department staff has access to a number of drug information resources, including many online references accessible from department computers. An on-site drug information center provides support for more in-depth questions.
Preferred Qualifications
Role Purpose
Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.
Responsibilities
Assess the overall status of the patient?s health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.
Participates in professional development and research activities.
Contributes to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.
Participates in the education and training of pharmacy staff, residents and students, and serves as a role model and mentor.
Participates in the identification and implementation of targeted drug cost reduction programs.
