SummaryThe Social Worker (Caregiver Support) is assigned to the Caregiver Support Program (CSP) with responsibility to work closely with CSP Leadership and/or designee to complete program required home visits - telephone contact - assessments - annual evaluations - and respond to Caregiver Hotline calls.
